By registering for and participating in the Run United 5k Run/Walk, I acknowledge and agree to the following:
I understand that participating in a road race or walk invoves physical activity that may be strenuous and carries inherent risks, including but not limited to falls, contact with other participants, traffic, weather conditions, and medical emergencies.
I certify that I am physically fit and sufficiently trained to participate in the this event. I assume all risks associated with participation.
In consideration of being permitted to participate in the Run United 5K, I hereby waive, release, and discharge Run United, event organizers, sponsors, volunteers, the City of Middleton, WI, and all affiliated parties from any and all claims, liabilities, damages, or causes of action arising out of my participation.
I grant permisssion for event organizers to use photographs, video recordings, or likenesses of me taken during the event for promotional purposes without compensation.
If registering a minor, I certify that I am the parent/legal guardian and consent to their participation under these same terms.
